Plan Notices & Disclosures

Dear APCO Employees:

We are pleased to provide you with the various notices and disclosures related to the health plan (the “Plan”). Please carefully review the information contained in the various notices and disclosures and share them with your covered dependents.  In the event of a conflict between the official Plan Documents and these legal notices or any other communication related to the Plan, the official Plan Document will govern.

 
There are several ways you can easily review the various notices and disclosures. These are described below:

 

• Review the notices here and below and download and print copies.

• Call APCO Human Resources or email benefits@apcoholdings.com and request that copies be mailed or emailed to you. 

​

• Visit HR/Benefits and ask to see copies. The office will obtain the requested materials and then supply them to you. 

​

• Call the insurance carrier at the phone number printed on your personalized member ID card and request that a benefit booklet be mailed or emailed to you.
